Suivi prospectif d'une cohorte d'infections ostéo-articulaires de l'enfant : validation d'un protocole d'antibiothérapie

Chirurgie générale, Introduction : les infections ostéo-articulaires de l’enfant constituent une urgence pédiatrique pouvant avoir un retentissement redoutable sur le pronostic articulaire et sur la croissance résiduelle. A ce jour, seuls des avis d’experts ont été proposés concernant le traitement en raison du manque d’étude clinique de qualité. L’objectif de notre étude était de rechercher des facteurs pronostics de mauvaise évolution. Méthode : nous avons mené une étude de cohorte prospective observationnelle monocentrique entre Janvier 2017 et Juin 2018 incluant les enfants admis pour infection ostéo-articulaire. Ils ont tous été traités selon notre protocole d’antibiothérapie intraveineuse initiale immédiatement après réalisation d’une ponction-lavage à l’aiguille en urgence en cas d’arthrite, ou d’emblée avant obtention des examens d’imagerie complémentaires dans les autres cas lorsque ceux-ci ne pouvaient être obtenus en urgence. Le relais antibiotique oral était effectué après obtention de l’apyrexie avec une CRP ≤ 20 mg/L. Résultats : 79 patients ont été inclus dont 29 cas d’arthrite, 46 cas d’ostéomyélite et 4 cas de spondylodiscite. 29 patients ont reçu une antibiothérapie IV de 5 jours et 21 patients ont reçu 4 jours ou moins. Le délai de normalisation de la température est de 3,13 jours, IC95%[1,98 – 4,27] et le délai d’obtention d’une CRP ≤ 20 mg/L est de 3,77 jours, IC 95% [2,64 – 4,90]. Dans le groupe des évolutions favorables, le délai de consultation était de 5,20 ± 1,84 alors que dans le groupe défavorable, il était de 11,36 ± 12,30, p-0,001. Conclusion : le facteur essentiel mis en évidence est le délai avant la consultation, ce qui confirme le caractère urgent du diagnostic afin d’instaurer rapidement une antibiothérapie intraveineuse initiale. Un traitement IV court de 4 (voire 3 jours) semble également être possible. Un traitement per os exclusif semble prometteur dans les formes non compliquées, permettant de réduire la morbidité hospitalière et le coût économique.
