Pathologies parodontales pédiatriques : spécificités cliniques, aspects interceptifs et thérapeutiques

Description : Médecine. Chirurgie dentaire, Les pathologies parodontales ou parodontopathies figurent parmi les maladies humaines les plus répandues, affectant jusqu’à 50% de la population mondiale. Elles peuvent être définies comme des maladies infectieuses multifactorielles, touchant les tissus mous et durs de soutien de la dent. À la suite d’une réunion de consensus organisée par la Fédération Européenne de Parodontologie et l’Académie américaine de Parodontologie, une nouvelle classification des conditions saines et pathologiques en parodontologie a été établie, ayant pour but d’actualiser la classification de 1999. Si les pathologies parodontales sont très largement abordées chez l’adulte, elles le sont beaucoup moins chez l’enfant et l’adolescent, bien que leur prévalence et leur sévérité demeurent élevées, avec des spécificités pédiatriques en termes de variabilité d’expression et de prise en charge. Les formes syndromiques de parodontopathies d’étiologie génétique, ainsi que celles associées à des pathologies onco-hématologiques ou immunitaires sont également à considérer. Dans une première partie, nous aborderons les particularités anatomiques et physiologiques du parodonte de l’enfant. Il est primordial de connaître l’aspect du parodonte sain, ainsi que son évolution et ses modifications avec l’âge, pour pouvoir faire le diagnostic différentiel avec un état pathologique. Ensuite, nous décrirons les conditions parodontales pathologiques retrouvées chez l’enfant, selon la nouvelle classification. Quatre grandes catégories émergent de cette classification: la santé parodontale et les maladies gingivales, la parodontite, les autres pathologies affectant le parodonte, et les conditions péri-implantaires saines et pathologiques, dans le contexte des thérapeutiques implantaires précoces. Outre les traitements parodontaux chez l’enfant, nous mettrons l’accent sur l’importance de la prévention et l’éducation à l’hygiène bucco-dentaire qui bien menées, peuvent aboutir à des résultats remarquables.
