Thromboprophylaxie en chirurgie orthopédique : étude de pratique en centre hospitalo-universitaire

Description : Médecine. Chirurgie générale, Introduction : l’anticoagulation est sujette à débat depuis de nombreuses années entre les différentes sociétés savantes. Le juste équilibre entre la prévention du risque thrombotique et le risque hémorragique iatrogène est à l’origine des recommandations nationales et internationales. Cette vérité n’est pas fixe et à vocation à évoluer dans le même sens que les améliorations des prises en charges chirurgicales. Quand était-il de l’application de ces recommandations au CHU Strasbourg en 2018 ? Dans cette étude, l’hypothèse était que nos pratiques étaient conformes aux recommandations dans plus de 70% des cas. Matériel et méthode : nous avons mené une étude observationnelle rétrospective, monocentrique et multi-opérateurs sur l’année 2018. Les dix interventions les plus fréquemment réalisées ont été sélectionnées. Par l’étude des dossiers informatiques nous avons recueillis l’ensemble des données démographiques et peri-opératoires de chaque patient opéré. La survenue de complications hémorragiques ou thrombotiques dans la période post-opératoire précoce (3 mois) a été étudiée. Nous avons retenu les recommandations nationales disponibles en 2018 (SFAR 2011) pour les comparer à nos pratiques locales. Une étude de conformité des prescriptions a été réalisée dans la population générale et pour chaque type d’intervention. De plus, un groupe arthroplastie (PTH + PTG) a également été constitué. Résultats : au total, 1528 interventions ont été inclues. Elles concernaient 1414 patients. Après échantillonnage, nous avons étudié 1117 dossiers, répartis-en 11 groupes. Dans 388 cas, il n’y avait pas d’indication d’anticoagulation post-opératoire.Dans l’effectif global, la conformité des prescriptions d’anticoagulant était significativement inférieure aux 70% attendus (57,21% ; IC 95% {0,0-0,59} ; p-0,0001). En sous-groupe, on retrouve ce résultat dans 5 situations (sous-groupe arthroplastie, PTH, PTG, changement de PTH et méniscectomie). Pour les groupes OTV, PTE et changement de PTE, les effectifs étaient trop faibles pour réaliser ce test statistique, mais les données montrent des conformités respectives de 92%, 100% et 100%. Enfin, pour les trois derniers groupes (changement de PTG, suture méniscale et LCA) la conformité des prescriptions n’était pas significativement inférieure aux 70% de notre hypothèse. Notons que cette non-conformité s’est faite en grande majorité par excès de prescription dans 40,56% des dossiers (+12,31 jours en moyenne, ET=6,24j). L’étude de la survenue des complications a dénombré 132 évènements hémorragiques post-opératoires dont 105 sont classés comme significatifs. Seuls 6 évènements thrombotiques ont été diagnostiqués. Conclusion : cette étude nous révèle qu’en dépit de recommandations nationales officielles, la tendance générale tend vers un excès de prescription d’anticoagulant en post opératoire en chirurgie orthopédique. Cet excès est assumé dans l’objectif de réduire l’incidence des évènements thrombotiques majeurs. Or, avec l’amélioration des prises en charge peri-opératoires, l’incidence de ces évènements diminue. Le taux de complication hémorragique dépasse de nos jours largement celui des thromboses. Cette étude confirme donc la nécessité de réévaluer le rapport bénéfice-risque de ces prescriptions et les nouvelles recommandations de la SFAR qui vont en ce sens.
